The ischemic cascade illustrates the course from ischemia to ECG changes and clinical symptoms. As illustrated, clinical symptoms (e.g chest pain) are the last signs of myocardial ischemia. It follows that myocardial ischemia may be present even in the absence of clinical symptoms and the ECG actually detects more ischemic episodes than the symptoms indicate.
