THRIVE Score for Stroke Outcome

Predicts outcome and mortality after acute ischaemic stroke.

THRIVE Score for Stroke Outcome
NIHSS
Age
History of hypertension
History of diabetes mellitus
History of atrial fibrillation
Result0 points

THRIVE 0-2. Good outcome (mRS 0-2) in roughly half or more; low mortality.

Risk group
THRIVE 0-2

When to use

  • Estimating likelihood of good functional outcome and mortality after acute ischaemic stroke, including after endovascular therapy.

Formula

Points: NIHSS (0-10 = 0, 11-20 = 2, 21-42 = 4), age (<=59 = 0, 60-79 = 1, >=80 = 2), and one point each for hypertension, diabetes, and atrial fibrillation. Range 0-9; higher scores predict worse outcome.

Pearls and pitfalls

  • Higher THRIVE scores track with lower rates of good functional outcome and higher mortality across treated and untreated cohorts.
