Simplified Autoimmune Hepatitis (AIH) Score

Diagnostic score for autoimmune hepatitis using antibodies, IgG, histology and exclusion of viral hepatitis.

Simplified Autoimmune Hepatitis (AIH) Score
ANA or SMA titer (or LKM ≥1:40, or SLA positive)
Points for antibodies do not add across ANA/SMA/LKM/SLA — maximum 2 points total for this category.
Serum IgG level
Liver histology
Absence of viral hepatitis (negative viral markers)
Result0 points

AIH unlikely.

Category
AIH unlikely

When to use

  • Supporting the diagnosis of autoimmune hepatitis in patients with compatible clinical, serological and histological findings.

Formula

Sum of: autoantibody titer (0-2, non-additive across ANA/SMA/LKM/SLA) + IgG level (0-2) + liver histology (0-2) + absence of viral hepatitis (0 or 2). Range 0-8: ≥7 definite AIH, 6 probable AIH, ≤5 unlikely.

Pearls and pitfalls

  • Designed to be simpler and more specific (though less sensitive) than the original 1999 IAIHG descriptive scoring system.
