When to use
- Estimating symptomatic intracranial haemorrhage risk in ischaemic stroke patients receiving IV alteplase.
Formula
Sum of: glucose 145-216 mg/dL = 1 or >216 = 2; early infarct signs on CT = 1; hyperdense artery sign = 1; age >75 = 1; NIHSS >=10 = 1. Range 0-6; higher scores carry progressively higher symptomatic ICH risk.
Pearls and pitfalls
- Applies to symptomatic haemorrhage; it does not by itself contraindicate thrombolysis.
- Blood sugar contributes up to 2 points, the only two-point item.
