When to use
- Prognosticating and triaging patients with Stevens-Johnson syndrome / toxic epidermal necrolysis, ideally on day 1 and day 3.
Formula
One point each for: age >=40, malignancy, heart rate >=120, epidermal detachment >10% BSA, BUN >28 mg/dL, glucose >252 mg/dL, bicarbonate <20 mEq/L. Mortality: 0-1 = 3.2%, 2 = 12.1%, 3 = 35.3%, 4 = 58.3%, >=5 = 90%.
Pearls and pitfalls
- Best measured within the first 24 hours and again at 72 hours, as detachment often progresses.
- SCORTEN can underestimate risk very early when detachment is still evolving.
