SCOFF Questionnaire
Five-item screen for anorexia nervosa and bulimia nervosa.
SCOFF Questionnaire
Do you make yourself SICK because you feel uncomfortably full?
Do you worry you have lost CONTROL over how much you eat?
Have you recently lost more than ONE stone (about 6.35 kg / 14 lb) in a 3-month period?
Do you believe yourself to be FAT when others say you are too thin?
Would you say that FOOD dominates your life?
Result0 points
Negative screen — an eating disorder is less likely, but clinical suspicion should still be pursued if present.
When to use
- Quick screen for anorexia nervosa and bulimia nervosa in primary care or general medical settings.
Formula
SCOFF = number of "yes" answers to the 5 items (Sick, Control, One stone, Fat, Food). Range 0-5; a score of 2 or more is a positive screen.
Pearls and pitfalls
- A positive screen indicates the need for further assessment, not a diagnosis on its own.
