When to use
- Identifying severe community-acquired pneumonia and the potential need for intensive care at presentation.
Formula
Major criteria: arterial pH <7.30 = 13, systolic BP <90 = 11. Minor criteria: RR >30 = 9, PaO2 <54 or PaO2/FiO2 <250 = 6, confusion = 5, BUN >30 mg/dL = 5, multilobar/bilateral infiltrates = 5, age >=80 = 5. A total >=10 marks high risk.
Pearls and pitfalls
- Also called the PS-CURXO-80 rule; the two acidosis/hypotension items carry the greatest weight.
- Presence of either major criterion alone reaches the high-risk threshold.
