Child up to 4 years of age
Symptoms present for at least 1 month
Two or fewer defecations per week
History of excessive stool retention
History of painful or hard bowel movements
History of large-diameter stools
Presence of a large fecal mass in the rectum
At least 1 episode/week of fecal incontinence after acquisition of toileting skills, and/or history of large-diameter stools that may obstruct the toilet (if toilet trained)

When to use
- Child up to age 4 years with suspected constipation, to confirm the diagnosis clinically without routine imaging or labs.
Formula
In a child up to age 4, for at least 1 month, ≥2 of: ≤2 defecations/week; excessive stool retention; painful or hard bowel movements; large-diameter stools; large fecal mass in the rectum; ≥1 incontinence episode/week or toilet-obstructing stools after toileting skills are acquired.
Pearls and pitfalls
- Alarm features (delayed meconium passage >48h, ribbon stools, blood in stool without fissure, failure to thrive, fever, bilious vomiting) should prompt evaluation for Hirschsprung disease or another organic cause rather than this diagnosis.
