Rome IV Diagnostic Criteria for Functional Dyspepsia

Symptom-based diagnostic criteria for functional dyspepsia and its subtypes.

Rome IV Diagnostic Criteria for Functional Dyspepsia
Bothersome postprandial fullness
Bothersome early satiation
Bothersome epigastric pain
Bothersome epigastric burning
Symptoms present for the last 3 months, with onset ≥6 months before diagnosis
No structural disease (including at upper endoscopy) likely to explain the symptoms
ResultCriteria not met

Rome IV criteria for functional dyspepsia are not fulfilled; consider structural/organic causes or an alternative functional GI disorder.

When to use

  • Diagnosing functional dyspepsia and distinguishing postprandial distress syndrome from epigastric pain syndrome after exclusion of structural disease.

Formula

At least one of bothersome postprandial fullness, early satiation, epigastric pain or epigastric burning, present for the last 3 months with symptom onset ≥6 months before diagnosis, and no structural explanation on evaluation (including upper endoscopy).

Pearls and pitfalls

  • PDS and EPS may overlap in the same patient and are not mutually exclusive.
