Abdominal pain occurring ≥4 days per month
Pain related to defecation
Pain associated with a change in stool frequency
Pain associated with a change in stool form (appearance)
If the child also has constipation: pain does not resolve with resolution of the constipation
Mark Yes if the child does not have constipation, or if pain persists despite constipation resolving.
Criteria fulfilled for at least 2 months prior to diagnosis
Symptoms not fully explained by another medical condition after appropriate evaluation

When to use
- Diagnosing IBS in a child or adolescent with recurrent abdominal pain and altered bowel habits.
Formula
Abdominal pain ≥4 days/month for ≥2 months, associated with ≥1 of: related to defecation, change in stool frequency, change in stool form; if constipation coexists, pain must not resolve with its resolution; no other explanation.
Pearls and pitfalls
- If pain resolves entirely with treatment of coexisting constipation, the correct diagnosis is functional constipation, not IBS.
