Symptoms present at least once per week for the last 1 month
2 or fewer defecations per week (developmental age ≥4 years)
≥1 episode of faecal incontinence per week
History of retentive posturing or excessive volitional stool retention
History of painful or hard bowel movements
Large faecal mass palpable in the rectum
History of large-diameter stools that may obstruct the toilet
Insufficient criteria for irritable bowel syndrome
Symptoms not fully explained by another medical condition after appropriate evaluation

When to use
- Diagnosing functional constipation in a child or adolescent (developmental age ≥4 years) without alarm features.
- Distinguishing functional constipation from organic causes before initiating empiric management.
Formula
Must include ≥2 of: ≤2 defecations/week, ≥1 faecal incontinence episode/week, retentive posturing/stool withholding, painful or hard bowel movements, large faecal mass in rectum, large-diameter stools — occurring at least weekly for ≥1 month, with insufficient criteria for IBS and no other explanation after evaluation.
Pearls and pitfalls
- Rectal examination and fecal mass are optional but supportive; the diagnosis can be made on history alone.
- Alarm features (weight loss, bilious vomiting, blood in stool, failure to thrive, late-onset after birth) warrant further work-up before labelling as functional.
