Abdominal discomfort or pain present for at least 12 weeks (need not be consecutive) in the preceding 12 months
Pain is relieved with defecation
Onset associated with a change in frequency of stool
Onset associated with a change in form (appearance) of stool

When to use
- Applying the original Rome consensus definition of IBS, chiefly of historical or comparative interest since Rome IV is now the preferred current criteria set.
Formula
At least 12 weeks (need not be consecutive) in the preceding 12 months of abdominal discomfort or pain plus 2 or more of: relief with defecation; onset associated with a change in stool frequency; onset associated with a change in stool form.
Pearls and pitfalls
- Rome II was superseded first by Rome III and then Rome IV, which tightened the duration/frequency requirements and replaced "discomfort" with pain-based wording.
