RiskE Score for Cardiac Surgery in Active Infective Endocarditis
Age
Prosthetic valve endocarditis
Staphylococcus aureus or fungal infection
Septic shock
Thrombocytopenia (<150,000/mm³)
Acute renal insufficiency
Cardiogenic shock
Periannular complication (abscess, pseudoaneurysm, fistula, prosthetic dehiscence)
Result3 % predicted mortality
Estimated in-hospital mortality with cardiac surgery for active left-sided infective endocarditis.
- RiskE points
- 0
- Predicted in-hospital mortality
- 3 %
When to use
- Estimating operative mortality risk when planning cardiac surgery for active left-sided infective endocarditis.
Formula
Sum weighted points (age, prosthetic infection, virulent organism, septic shock, thrombocytopenia, acute renal insufficiency, cardiogenic shock, periannular complication; range 0-68); mortality % = logistic transform of the total score, calibrated to the published anchor mortalities of 3% at 0 points, 40% at 30 points, and 97% at 68 points.
Pearls and pitfalls
- Outperformed EuroSCORE, STS-IE, and PALSUSE for discrimination in the derivation/validation cohorts.
