When to use
- Grading severity of acute kidney injury (Risk, Injury, Failure).
Formula
Risk: creatinine >=1.5x baseline or UO <0.5 mL/kg/h x6 h. Injury: >=2x or UO <0.5 x12 h. Failure: >=3x (or creatinine >=4 mg/dL) or UO <0.3 x24 h / anuria x12 h. The worse of the two criteria defines the class.

- Loss and ESKD (end-stage) categories are time-based outcomes (>4 weeks and >3 months) and are not computed here.
