When to use

  • On the first day of hospitalization for acute myocardial infarction, to flag patients for early targeted readmission-prevention interventions.

Formula

Points: creatinine >2 mg/dL (+6), elevated BNP/NT-proBNP (+8), age (+1 per decade above 18, i.e. floor((age-18)/10)), diabetes mellitus (+4), female sex (+2), no timely PCI within 24 h (+1), systolic BP <100 mmHg (+3). Score <=13 low risk (~2% readmission), >=20 high risk (~34% readmission).

Pearls and pitfalls

  • Derived and validated in 826 AMI hospitalizations across 6 north Texas hospitals; not externally validated in other populations.
  • Designed so every input is available within the first 24 hours of admission, enabling early targeting of transitional-care resources.
