REACH-B Score for Hepatocellular Carcinoma (HCC)
Estimates future HCC risk in non-cirrhotic patients with chronic hepatitis B.
REACH-B Score for Hepatocellular Carcinoma (HCC)
Sex
Age
Derived cohort was recruited between ages 30 and 65.
ALT (alanine aminotransferase)
HBeAg status
HBV DNA level
Result0 points
Lower predicted cumulative incidence of HCC at 3, 5 and 10 years; risk rises steeply with increasing total score in the original nomogram.
- Score range
- 0–17
When to use
- Estimating future HCC risk in non-cirrhotic, treatment-naive patients with chronic hepatitis B to guide surveillance intensity.
Formula
Sum of points for sex, age band, ALT, HBeAg status and HBV DNA level; total range 0-17. Consult the original publication for the full score-to-risk lookup table.
Pearls and pitfalls
- Developed and validated in Asian cohorts without cirrhosis; underestimates risk in cirrhotic or antiviral-treated patients (prompting modified versions such as mREACH-B).
