When to use

  • Risk-stratifying acutely unwell adults with suspected COVID-19 in the emergency department.

Formula

NEWS2 physiology (respiratory rate, SpO2, supplemental O2, systolic BP, heart rate, temperature, consciousness) plus age (0/2/3/4), sex (male +1), and WHO performance status (0-4). Total 0-29 maps to 30-day risk of death or organ support.

Pearls and pitfalls

  • A threshold of >4 points is highly sensitive but poorly specific for adverse outcome.
  • Uses NEWS2 SpO2 Scale 1; a different scale applies to patients with chronic hypercapnic respiratory failure.
