When to use

  • Estimating ICU mortality before initiating veno-venous ECMO for severe ARDS.

Formula

Points from MAP (0-4), lactate (0-4), arterial pH (0-3), platelets (0-2), pre-ECMO hospital days (0-2). Range 0-15. Class I 0-5, II 6-9, III 10-15.

Pearls and pitfalls

  • Built from extrapulmonary predictors; it complements rather than replaces lung-focused scores such as RESP.
