When to use
- Estimating short- and longer-term prognosis at the bedside shortly after admission for acute ischaemic stroke.
Formula
PLAN = age/10 + 1.5 (pre-stroke dependence) + 1.5 (cancer) + 1 (congestive heart failure) + 1 (atrial fibrillation) + 5 (reduced level of consciousness) + 2 (significant/total arm weakness) + 2 (significant/total leg weakness) + 1 (neglect or aphasia).
Pearls and pitfalls
- Derived and validated in the Registry of the Canadian Stroke Network using only variables available at admission.
- Should augment, not replace, decisions about reperfusion therapy.
