PE-SARD bleeding score for acute pulmonary embolism

Early major bleeding risk in acute PE (syncope, anaemia, renal dysfunction).

PE-SARD bleeding score for acute pulmonary embolism
Syncope at presentation
Anaemia (haemoglobin <12 g/dL)
Renal dysfunction (eGFR <60 mL/min/1.73 m2)
Result0.0 points

Low risk; early (30-day) major bleeding about 1 %.

Risk category
Low risk
Early major bleeding
about 1 %

When to use

  • Estimating early major bleeding risk when starting anticoagulation for acute PE.

Formula

Sum of points: syncope 1.5, anaemia (Hb <12 g/dL) 2.5, renal dysfunction (eGFR <60) 1. Range 0-5. Low = 0, intermediate = 1-2.5, high = >2.5.

Pearls and pitfalls

  • Weighs bleeding risk, not PE mortality; pair with a severity score such as sPESI when deciding disposition.
