When to use

  • Intraoperative/postoperative estimation of clinically relevant pancreatic fistula risk after pancreatoduodenectomy, to guide drain management and perioperative decisions.

Formula

Sum of: gland texture (hard 0, soft 2) + pathology (adenocarcinoma/pancreatitis 0, other 1) + duct diameter (≥5mm 0 to ≤1mm 4) + blood loss (≤400mL 0 to >1000mL 3). Range 0-10; negligible 0, low 1-2, moderate 3-6, high 7-10.

Pearls and pitfalls

  • Blood loss and gland texture were not confirmed as independent predictors in every external validation, prompting alternative scores (e.g. a-FRS) that omit blood loss.
