Weighted sum: active cancer, prior VTE, reduced mobility, thrombophilia (3 each); recent trauma/surgery (2); age ≥70, cardiac/respiratory failure, MI/stroke, infection/rheumatological disease, obesity, hormonal therapy (1 each). High risk ≥4.
