Nutrition Risk Screening 2002 (NRS-2002)
Screens hospitalized adults for malnutrition risk by combining nutritional status and disease severity.
Nutrition Risk Screening 2002 (NRS-2002)
Impaired nutritional status
Severity of underlying disease
Age >=70 years
Result0 points
Patient is not currently at nutritional risk; re-screen weekly.
When to use
- Screening hospitalized adults on admission to determine who needs a formal nutrition care plan.
Formula
NRS-2002 = impaired nutritional status score (0-3) + disease severity score (0-3) + 1 point if age >=70. Range 0-7; >=3 indicates nutritional risk.
Pearls and pitfalls
- Designed for and validated in hospital inpatients, based on the pooled evidence from controlled nutrition-support trials.
- Preceded by a 4-item initial screen (low BMI, recent weight loss, reduced intake, serious illness); any positive answer triggers the full final screen shown here.
