When to use

  • Identifying ICU patients most likely to benefit from early, aggressive nutrition support, as opposed to those at low nutritional risk.

Formula

Modified NUTRIC (no IL-6) = age points (0-2) + APACHE II points (0-3) + SOFA points (0-2) + >=2 comorbidities (0-1) + days from hospital to ICU admission >=1 (0-1). Range 0-9; >=5 = high risk.

Pearls and pitfalls

  • The original NUTRIC score included IL-6 (range 0-10); the modified version omits it since IL-6 is not routinely measured and performs comparably.
  • A high score identifies risk of poor outcome, not simply the need for more calories - overfeeding should still be avoided.
