When to use

  • Non-invasive risk stratification for advanced fibrosis in patients with biopsy-proven or suspected NAFLD, to decide who needs further hepatology workup.

Formula

NFS = -1.675 + 0.037xage(yr) + 0.094xBMI(kg/m2) + 1.13x(IFG/diabetes, 1=yes) + 0.99x(AST/ALT) - 0.013xplatelets(x10^9/L) - 0.66xalbumin(g/dL).

Pearls and pitfalls

  • Score < -1.455 has high negative predictive value for advanced fibrosis; > 0.676 has high positive predictive value.
  • Not validated in patients with other chronic liver diseases or decompensated cirrhosis.
