When to use

  • Predicting survival specifically in secondary (post-polycythemia vera or post-essential thrombocythemia) myelofibrosis, where the primary-myelofibrosis-derived IPSS/DIPSS underperform.

Formula

Score = 0.15×age(yrs) + 2 (if Hb<11 g/dL) + 2 (if circulating blasts ≥3%) + 2 (if CALR-unmutated) + 1 (if platelets<150x10⁹/L) + 1 (if constitutional symptoms). Low <11, intermediate-1 11-<14, intermediate-2 14-<16, high ≥16.

Pearls and pitfalls

  • Outperformed IPSS, DIPSS, and DIPSS-plus for survival prediction in the original and validation cohorts of secondary myelofibrosis.
