Mutation-Adjusted Risk Score (MARS) for Advanced Systemic Mastocytosis

Estimates overall survival in advanced systemic mastocytosis using clinical and molecular data.

Mutation-Adjusted Risk Score (MARS) for Advanced Systemic Mastocytosis
Age >60 years
Hemoglobin <10 g/dL
Platelets <100 × 10⁹/L
High molecular risk mutations (SRSF2, ASXL1, and/or RUNX1)
Result0 points

Low risk (median OS not reached).

Risk group
Low risk

When to use

  • Prognostic stratification of patients with advanced systemic mastocytosis (aggressive SM, SM with an associated hematologic neoplasm, or mast cell leukemia).

Formula

MARS = age >60y (1) + hemoglobin <10 g/dL (1) + platelets <100×10⁹/L (1) + high molecular risk mutation burden (0 mutations = 0, 1 mutation in SRSF2/ASXL1/RUNX1 = 1, ≥2 mutations = 2). Range 0-5. Low risk 0-1, intermediate risk 2, high risk 3-5.
