When to use
- Assessing radiologic response of hepatocellular carcinoma after locoregional or systemic therapy, where necrotic (non-enhancing) tumor should not count as residual disease.
Formula
Only arterially-enhancing (viable) tumor is measured. CR = no enhancement in any target lesion; PR = ≥30% decrease in sum of viable diameters; PD = ≥20% increase or new lesion; SD = neither.
Pearls and pitfalls
- Designed to adapt RECIST for the fact that effective HCC therapies (TACE, ablation) devascularize tumor without necessarily shrinking its outer contour.
