Formula

Points: lumens (single 0 / double 1 / triple-quad 2 / >=5 3); prior VTE (>30d 2, <=30d 3); active cancer 3; other CVC 1; WBC >12 1. Class I=0, II=1-2, III=3-4, IV>=5.

Pearls and pitfalls

  • Higher lumen counts and concurrent catheters are the most modifiable inputs.
