0 = normal sleep; 6 = <2-3 hours sleep
Reduced appetite
0 = normal appetite; 6 = needs persuasion to eat
Concentration difficulties
0 = none; 6 = unable to read or converse
Lassitude
0 = none; 6 = complete inability to start simple tasks without help
Inability to feel
0 = normal interest in surroundings; 6 = complete anhedonia
Pessimistic thoughts
0 = none; 6 = delusions of ruin, remorse, or guilt
Suicidal thoughts
0 = enjoys life; 6 = explicit plans for suicide

When to use

  • Rating depression severity and monitoring response to antidepressant treatment, particularly sensitive to change over time.

Formula

Sum of 10 items, each rated 0-6. Range 0-60. Bands: 0-6 none, 7-19 mild, 20-34 moderate, >=35 severe.
