When to use
- Early identification of patients at risk of ARDS at hospital or ICU admission.
Formula
Sum of predisposing conditions (shock 2, aspiration 2, sepsis 1, pneumonia 1.5), high-risk surgery (orthopaedic spine 1, acute abdomen 2, cardiac 2.5, aortic vascular 3.5, +1.5 if emergency), high-risk trauma (TBI 2, smoke inhalation 2, near drowning 2, lung contusion 1.5, multiple fractures 1.5) and modifiers (alcohol 1, obesity 1, hypoalbuminaemia 1, chemotherapy 1, FiO2 >0.35 = 2, RR >30 = 1.5, SpO2 <95% = 1, acidosis 1.5, diabetes -1 only with sepsis). Cut-off >4.
Pearls and pitfalls
- Best discrimination at LIPS >4 (sensitivity ~69%, specificity ~78%).
- Diabetes is counted as a protective modifier (-1) only in the presence of sepsis.
