When to use

  • Assessing clinical activity of Takayasu arteritis based on new or worsening features over the preceding 3 months.
  • Tracking disease activity over time or in response to immunosuppressive therapy.

Formula

Sum of new or worse (within the past 3 months) clinical features across systemic, cutaneous, ocular, cardiovascular, abdominal, renal, and neurological domains. Bruit, pulse inequality, pulse loss, claudication, carotidodynia, new diastolic hypertension, and stroke each score 2 points; all other features score 1 point.

Pearls and pitfalls

  • This implementation covers the major validated domains and the documented weighting scheme (five cardiovascular items, one renal item, and one neurological item weighted at 2 points) rather than the full 44-item instrument used in the original publication; consult the original paper for the complete standardized item list used in formal trials.
  • An acute-phase-reactant-adjusted variant (ITAS-A, adding 0-3 points for ESR/CRP) has also been described.
