Prolongation of prothrombin time (PT)

Score <5: suggestive of (not affirming) non-overt DIC; repeat testing in 1-2 days.

When to use
- Patients with a disorder known to be associated with DIC (sepsis, trauma, malignancy, obstetric catastrophe) and abnormal coagulation studies.
Formula
Sum of: platelet count (0/1/2), fibrin-related marker elevation (0/2/3), PT prolongation (0/1/2), fibrinogen (0/1). Score >=5 compatible with overt DIC.
Pearls and pitfalls
- Intended only after confirming an underlying condition associated with DIC.
- Should be repeated serially, as the score can evolve over 24-48 hours.
