Revised International Prognostic Scoring System (IPSS-R) for MDS

Risk stratification for myelodysplastic syndrome using cytogenetics, blast percentage, and cytopenias.

Very low risk; published median overall survival approximately 8.8 years.

When to use

  • Risk-stratifying newly diagnosed MDS to guide prognosis discussion and treatment intensity (e.g. transplant timing).

Formula

IPSS-R = cytogenetic risk (0-4) + BM blast % (0-3) + hemoglobin (0-1.5) + platelets (0-1) + ANC (0-0.5). Risk categories: very low <=1.5, low >1.5-3, intermediate >3-4.5, high >4.5-6, very high >6.
