International Prognostic Score for Asymptomatic Early-stage CLL (IPS-E)
Predicts time to first treatment in early-stage (Binet A/Rai 0) CLL.
International Prognostic Score for Asymptomatic Early-stage CLL (IPS-E)
Unmutated IGHV gene
Absolute lymphocyte count >15 x10^9/L
Palpable lymph nodes
Result0 points — Low risk
Estimated 5-year cumulative risk of requiring treatment: about 8.4 %.
- 5-year risk of treatment
- 8.4 %
When to use
- Stratifying newly diagnosed, asymptomatic Binet stage A (or Rai 0) CLL for likelihood of needing treatment.
- Not intended for patients who already have an indication for treatment.
Formula
1 point each for unmutated IGHV, absolute lymphocyte count >15x10^9/L, and palpable lymph nodes. Low risk = 0, intermediate = 1, high = 2-3.
Pearls and pitfalls
- Derived and validated in >2,000 patients across eight cohorts; does not require FISH or TP53 status.
