Formula

Points: low haemoglobin 1, oncology discharge 2, low sodium 1, any procedure 1, non-elective admission 1, prior admissions (2-5)=2 or (>5)=5, LOS >=5 days=2. Categories: 0-4 low, 5-6 intermediate, >=7 high.
