When to use
- Estimating the need for a primary-prevention implantable cardioverter-defibrillator in patients with hypertrophic cardiomyopathy.
Formula
Prognostic index = 0.15939858 x MWT - 0.00294271 x MWT^2 + 0.0259082 x LA diameter + 0.00446131 x max LVOT gradient + 0.4583082 x family history SCD + 0.82639195 x NSVT + 0.71650361 x unexplained syncope - 0.01799934 x age. 5-year SCD probability = 1 - 0.998^exp(Prognostic index).
Pearls and pitfalls
- Not validated for age <16 years, prior SCD survivors, or infiltrative/metabolic phenocopies of HCM.
- ESC guideline bands: <4% low, 4 to <6% intermediate, >=6% high 5-year risk.
