When to use
- Risk-stratifying patients across the acute coronary syndrome spectrum (STEMI, NSTEMI, unstable angina) to guide triage and invasive strategy timing.
Formula
Sum of weighted points for age, heart rate, systolic BP, creatinine (each via piecewise bands), Killip class, cardiac arrest at admission, ST-segment deviation, and elevated cardiac biomarkers. Total score >140 = high risk, 109-140 = intermediate, <=108 = low risk of in-hospital death.
Pearls and pitfalls
- Developed and validated in the multinational GRACE registry across the full ACS spectrum, unlike TIMI which was derived in trial populations.
- The same eight variables also generate 6-month and post-discharge mortality estimates via separate nomograms.
