GIR (mg/kg/min) = [dextrose concentration (%) x infusion rate (mL/hr) x 10] / [weight (kg) x 60].
Pearls and pitfalls
- Typical maintenance GIR in neonates/infants is roughly 4-8 mg/kg/min; tolerance varies with gestational age and clinical status.
- Recalculate whenever the infusion rate, dextrose concentration, or weight changes.
