Glucose >10 mmol/L (>180 mg/dL), no history of diabetes
Urea >16 mmol/L (BUN >45 mg/dL) after IV fluids
PaO2 <8 kPa (<60 mmHg) on room air
Corrected calcium <2 mmol/L (<8 mg/dL)
LDH >600 IU/L (or AST/ALT >200 IU/L)
Albumin <32 g/L (<3.2 g/dL)

When to use
- Assessing severity of acute pancreatitis within the first 48 hours of admission to identify patients needing higher-level care.
Formula
PANCREAS mnemonic, 1 point each within 48h: PaO2 <8kPa, Age >55, Neutrophils (WBC) >15, Calcium <2mmol/L, Renal (urea) >16mmol/L, Enzymes (LDH >600 or AST/ALT>200), Albumin <32g/L, Sugar (glucose) >10mmol/L. Score >=3 = severe.
Pearls and pitfalls
- A modification of Ranson's criteria designed to be assessable without needing values only available at admission (e.g. does not require baseline haematocrit fall or fluid sequestration).
- Best applied at 48 hours after admission; an early single time-point score does not reflect trajectory.
