When to use

  • Estimating survival in primary myelofibrosis using cytogenetics and driver/high-molecular-risk mutations alone, e.g. when clinical/laboratory data for MIPSS70/DIPSS are unavailable.

Formula

Points: very high risk karyotype = 2, unfavourable karyotype = 1 (favourable = 0); absence of type 1/like CALR = 1; ASXL1, SRSF2, or U2AF1Q157 mutated = 1 each. Total 0-6: Low (0), Intermediate-1 (1), Intermediate-2 (2), High (>=3).

Pearls and pitfalls

  • Uses only genetic data (no clinical or laboratory variables), simplifying application when marrow cytogenetics and NGS are available.
