Any nodal or extranodal tumour mass >7 cm in diameter
Three or more nodal sites, each >3 cm in diameter
Systemic (B) symptoms
Symptomatic or organ-threatening splenomegaly
Compression syndrome (ureteral, orbital, gastrointestinal, etc.)
Ascites or pleural effusion
Leukemic phase (>5.0 x 10^9/L circulating lymphoma cells)
Cytopenia (ANC <1.0 x 10^9/L or platelets <100 x 10^9/L) due to lymphoma
ResultCriteria not met
No high tumour-burden criterion is present; expectant management ("watch and wait") can reasonably be considered if the patient is asymptomatic.

When to use
- Deciding between immediate treatment and expectant observation in newly diagnosed, asymptomatic follicular lymphoma.
Formula
Any single criterion present (tumour mass >7 cm; >=3 nodal sites >3 cm; B symptoms; symptomatic splenomegaly; compression syndrome; ascites/pleural effusion; leukemic phase; or lymphoma-related cytopenia) defines high tumour burden.
Pearls and pitfalls
- Developed in the pre-rituximab era but still widely used to select trial populations and guide the watch-and-wait decision.
