When to use
- Antidotal therapy for confirmed or suspected methanol or ethylene glycol poisoning.
Formula
Loading 15 mg/kg IV. Maintenance 10 mg/kg every 12 h for 4 doses, then 15 mg/kg every 12 h (fomepizole induces its own metabolism). Give every 4 h during haemodialysis.
Pearls and pitfalls
- Escalation to 15 mg/kg after 48 h accounts for autoinduction of metabolism.
