When to use
- Accelerated diagnostic pathway for possible cardiac chest pain in the ED.
Formula
Age points (+2 for 18-45, +2 per additional 5-year band up to +20); male +6; known CAD or (18-50 and >=3 risk factors) +4; diaphoresis +3; radiation to arm/shoulder/neck/jaw +5; pleuritic pain -4; reproducible on palpation -6. Low risk = EDACS <16 plus non-ischaemic ECG and negative serial troponin.
Pearls and pitfalls
- The low-risk designation requires the score AND a normal ECG AND negative troponins.
- Risk factors: family history, dyslipidaemia, hypertension, diabetes, current smoking.
