When to use
- Quantifying atopic dermatitis extent and severity at baseline and over time, including as a clinical trial endpoint (e.g., EASI-75).
Formula
For each of 4 body regions (head/neck, trunk, upper limbs, lower limbs): region score = (erythema + induration/papulation + excoriation + lichenification, each 0-3) x area score (0-6) x region weighting factor. Adult factors: head 0.1, trunk 0.3, upper limbs 0.2, lower limbs 0.4. Factors for age <8y: head 0.2, trunk 0.3, upper limbs 0.2, lower limbs 0.3. Total EASI = sum of the 4 region scores (range 0-72).
Pearls and pitfalls
- Excoriation and lichenification cannot be scored in the absence of any erythema, induration, or papulation at that site per the original methodology, but the simplified additive formula is used clinically as a close approximation.
- EASI-50/EASI-75/EASI-90 (percentage reduction from baseline) are the standard trial response thresholds.
