When to use

  • Estimating exercise capacity from a brief self-administered questionnaire when formal cardiopulmonary exercise testing is unavailable.
  • Supporting preoperative risk discussions ahead of intermediate- or high-risk noncardiac surgery.

Formula

DASI = sum of weighted points for 12 activities of daily living (range 0-58.2). Estimated peak VO2 (mL/kg/min) = 0.43 x DASI + 9.6; METs = VO2/3.5.

Pearls and pitfalls

  • A self-reported instrument; it correlates with, but does not replace, measured exercise capacity.
  • A cutoff near 4 METs is commonly used in perioperative guidelines to flag patients warranting further cardiac evaluation.
