When to use

  • Grading disease activity in Crohn's disease and tracking response to therapy, typically also used as an entry/response criterion in clinical trials.

Formula

CDAI = 2x(liquid/soft stools, 7-day sum) + 5x(abdominal pain, 7-day sum) + 7x(general well-being, 7-day sum) + 20x(number of complication categories) + 30x(antidiarrheal use, 0/1) + 10x(abdominal mass: 0/2/5) + 6x(47-Hct for men or 42-Hct for women) + 1x(% below standard weight).

Pearls and pitfalls

  • CDAI <150 = remission, 150-219 = mild, 220-450 = moderate-severe, >450 = severe.
  • Requires a 7-day symptom diary; not practical as a single-visit bedside tool.
