When to use

  • Distinguishing immune (alloimmune) from non-immune (consumptive) causes of a poor platelet transfusion response.

Formula

CCI = (post-transfusion count − pre-transfusion count [/µL]) × body surface area (m²) / number of platelets transfused (×10¹¹).

Pearls and pitfalls

  • A CCI ≥7500 at 1 hour or ≥4500 at 24 hours is generally considered an adequate response.
  • Two consecutive inadequate CCIs on ABO-matched fresh platelets define refractoriness and warrant HLA antibody testing.
