When to use
- Estimating overall comorbidity burden and 10-year survival for prognostic discussions, research risk-adjustment, or preoperative planning.
Formula
Weighted sum of comorbidities (1 point: MI, CHF, PVD, CVA/TIA, dementia, COPD, connective tissue disease, peptic ulcer disease, mild liver disease, uncomplicated diabetes; 2 points: hemiplegia, moderate-severe renal disease, diabetes with end-organ damage, localised tumour, leukaemia, lymphoma; 3 points: moderate-severe liver disease; 6 points: metastatic tumour, AIDS) plus age points (0 for <=40, +1 per decade to +5 for >80). Estimated 10-year survival = 0.983^(e^(total score x 0.9)) x 100.
Pearls and pitfalls
- The age-adjusted survival formula is an approximation from the original derivation cohort and should not replace individualised prognostication.
