When to use

  • Grading confirmed primary blast lung injury to anticipate ARDS and guide ventilation.

Formula

Take the worst of: PaO2/FiO2 (>200 mild, 60–200 moderate, <60 severe), chest film (localised/diffuse/massive), and bronchopleural fistula (present raises to at least moderate).
